Teamspeak Staff Incident Report:
Post Name Should be
“Your Name | Incident Report # 1 / 2 / etc”
This is a form which is not entirely formal, however as teamspeak staff you are required to write down short descriptive blurbs about situations you’ve dealt with once a week Failure to write this report at least once a week will result in demotion. I recommend you keep a notepad document of things you deal with saved somewhere.
A report can be as simple as shown below

User Gary was being disrespectful to users in the public gaming lounge and received a kick then a short 1 hour ban after he continued.
Sparks was seen abusing his teamspeak power and was reported to the Deputy Head TS Admin Skips to be dealt with.

Joey had a disrespectful name “Sparks is gay” and was told to change it, he complied and I have not had any further problems or complaints about him.
